Release of Liability As the legal parent or guardian, I release and hold harmless Broadway Bound Dance Center, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control of Broadway Bound Dance Center, its owners and operators or in route to or from any of said premises. I have read and understand and agree to release of liability Photo Release Broadway Bound Dance Center is hereby granted permission to take photographs of the students to use in brochures, web sites, posters, advertisements, and other promotional materials the school creates. I understand the student’s name may be used in conjunction with a picture for studio newsletters / programs only. I also understand the student’s name will not be published on any other materials without prior parental permission. Permission is hereby granted for Broadway Bound to copyright such photographs in its name.
